The
dermis (outer layer of the skin) is made up of strong fibres that connect to
each other and allow your skin to stretch as you grow. However, if your skin
doesn’t bounce back after it's been stretched while growing or due to
pregnancy, rapid weight gain, extreme weight loss, or use of toning and
bleaching products. Hence, you tend to have scarring called stretch marks.
It
tends to affect women more than men and are often consider a source of shame.
Stretch marks are unflattering and cause drop in self-esteem, so in a bid to
look superb, people spend lots of money getting creams and lotion that still
won’t solve the scarring.
So

TREATMENT
OPTIONS
We
use the fraxel or fractional laser or simply called the CO2 in the treatment of
stretch mark.
With
a fraxel or CO2 laser, you are sure of the result of treatment. This laser
works on two principles which is removing some layer of the skin involved and
also sending heat to the dermis to stimulate more collagen formation.
The
first few days will cause reddening and darkening of the area involved with
exfoliation and discomfort.
Subsequently
there will be total removal of the stretch mark in the involve area because new
skin formation and collagen formation in the involved area.
The
down time is between 2-4 weeks for every treatment and you are sure of a clear
new skin. This procedure requires multiple sessions especially if they are old
stretch marks.
The
areas treated with the CO2 laser or fraxel laser might be sore for some days
but the skin strength and integrity is restored back in no time. Other
functions of the CO2 laser include vaginal tightening, acne scars, post burn
scars, keloids, hypertrophic scars, wrinkles, pigment removal, skin resurfacing

Over
the years, wound has plague humanity and many methods of treatment has been
developed to modify the challenges that have affected the human system. Over
the years many have defined wound as either break in the epithelial lining of
the surface of an organ or tissue while some feel wound is simple an ulcer.
It
became obvious to me that not many articles has been published or written for
the public to understand the concept of wound; People don't really know where,
who and how to get wound care. Therefore, we have decided to write a little on
wound care for the public to understand
In
developing countries, it’s been a big challenge of whom and how to refer a
patient with wound care in both primary health centers and other referral
centers. Many are not so particular on the care of wound because of the
complexity and the challenges of wound.
What
is wound?
In
standard Practice, wound is defined as a break in the structural integrity of a
tissue or an organ. Wound can be interchange with an ulcer but not all wound
are ulcer. Wound is damage in structural integrity of the tissue while in ulcer
it is a break in the lining of the tissue or organ e.g a closed contusion
injury of the skin which might not have an obvious injury but the integrity of
the tissue has been compromised this could be as a result of injury caused by
trauma or burn.
This
can also be looked into as direction of injury to organs like kidney or liver
where there could be closed injury to those organs without an outside
manifestation of problem.
Wound
can be applied to involve many organs in the body but for a lot of people,
wound is seen only affecting the skin. This can be well explained in injury
involving the skin like in thermal injury which involves the underlying
structure of the skin like the nerve or blood vessels as seen in the 4th degree
burns.
It
may appear simply as an abrasion, laceration, bruises, burn wound, open wound,
closed wound, contusion, concussion, maceration and so on.
Wound
Classification?
Many
methods have been used to classify wound but can simply be classified as;

Infected wound e.g. wound due to infection
2,Metabolic
wound e.g. diabetic ulcer
3,Venous
ulcers e.g. varicose vein ulcer which we often see on the inner ,lower part of
the legs
4,Malignant
ulcer e.g. ulcer or wound due to things like skin cancer. This can be seen on
any part of the body starting from the face to the leg. breast cancer ulcer,
skin cancer ulcer etc.
5,Arterial
ulcer-this is a wound as a result of poor / reduced blood flow to part of the body
especially the leg
6,Hematogenous
ulcer e.g. sickle cell anaemia ulcer, Commonly experienced by people with
sickle cell anaemia.
7,
Traumatic ulcer e.g. after fall or accident
8,
Burn ulcer or wound e.g. wound in people who sustained injury from burn
WHAT
YOU SHOULD AVOID
Any
suspicious wound should not be kept away from the doctor .Most times people with wound and burns visit pharmacy or
chemist and they prescribed antibiotics without proper investigations; This
could lead to micro bacteria resistant which could make antibiotics treatment
difficult for the same person in other infection difficult because of abuse of
antibiotics.
How
do you treat wound or ulcer?
In
the western society, there is wound specialist but most of their works varies
around wound in Nigeria the wound specialist are the Plastic Surgeons.
Often
times, people have this notion that plastic surgeons is meant for beauty, nose
and breast fixer but trust me, they do more than that in their daily
activities.
What
is liposuction?
Liposuctioning
is a minimally invasive surgery done to shape the body. Liposuction is a
minimal risk procedure which is safe and use in improving and reshaping of the
body. Liposuction does not require you spending days after the procedure, you
can return to daily activity as soon as you feel comfortable. The procedure is
not a whole day thing, its takes few hours or less and you’re ready to go. Fat
removed from one part of the body can be injected to other part where
contouring is desired (fat transfer).This same fat can serve as a good
substitute for prosthetics breast implants or silicon breast implant which we noticed n many especially the colored skin that they
are averse to it.
Areas
of concern for body sculpturing are the abdomen, thighs, breasts, hips, pubis,
buttocks, male breast tissue (gynaecomastia), chin/neck, back of the arms, the
love handles, the legs and the forehand.
How
we work
In
our clinic, we make use of the power assisted liposuction machine (2015 edition
Moller) with tumescent technique in liposuction and fat transfer. This
technique uses cannula that has a spinning end to break the fat while the fat
is being suctioned. In this procedure we use standard markings and incisions
less than 5mm to pass the cannula under the skin to the fat layer while the
cannula with a spinning end breaks the deep and superficial fats and
simultaneously suck the fats.
This
technique has been proven to reduce bruising and bleeding, less risk and
discomfort. Liposuction is usually done as an outpatient procedure in our
properly equipped clinic for localized areas of the body especially when there
is no other major procedure to be done. In general, it does not require an
overnight hospital stay unless a large volume of fat is being removed or
additional procedure is done. Local anesthesia is used in some cases and you
may or may not be given a sedative to help you relax. If a large area or volume
of fat is being removed, general anesthesia or deep sedation with a local
anesthetic may be used.
Our
fat transfer is by use of Tulip cannula an American device after careful
separation of the fat before transfer is done to request areas of the body e.g.
the butt or the face.
Acne
This
is a common skin problem that affects both young and old, either in the active
phase or reminiscences stage with scarring of the face, arm, back and shoulder.
While it might sound funny from our observation and experience, acne affects
teenagers and it is also common in people in their early age. Many adult are
also living with the consequence of acne they had in past or the complications
from the acne or as a result of products recommended by people or cosmetic
shops.
It
should be clear that a block or clog of the skin pore will result into acne
after the sebaceous gland secretion fails to get to the surface of the skin. In
addition there are also organisms in or on the skin that facilitate more oil
product with resultant effect of more acne or oil production. A lot of people
having active acne, in addition to excess oil secretion, while people look for
solution, many problems therefore occur as a result of using all sorts of skin
care products.
What
can be done?
Understanding
of what is good and appropriate for the face is very important. Treatment of

WHAT

Blood
moves from the heart through the arteries all the way to the capillaries. The
vein collects blood from the leg back to the heart. Veins could be superficial
(situated on or near the surface). In some cases it’s not noticeable.
Telangiectasia
is a condition in which widened venules (tiny blood vessels) cause threadlike
red lines or patterns on the skin. These patterns, or telangiectases, form
gradually and often in clusters. They’re sometimes known as “spider veins”
because of their fine and weblike appearance.
Normal
superficial vein can be seen on the skin, for a fair skin person, it appears
greenish. It could be obvious in fair or dark skin people especially when they
are using lightening creams. In some cases, there could be problem with deep or
superficial vein on the body.
Remember,
there can be physiologic condition that can make the veins obvious e.g
pregnancy. Majority of the cases, there are problem with either the superficial
or deep veins and therefore it makes them more obvious. Sometimes pregnancy
makes the condition worse.
INSTANCES/CASES
•
In some cases, there are defect with the veins even from birth or more obvious
while growing up which is noticed with progressive swelling of the legs.

In some cases, it can lead to skin discolouration and subsequently wound on the
leg.

Sometimes problem with this veins can manifest as small multiple veins on the
legs called Spider Veins or Telangiectasia
